The deadline to submit a Claims Form will be no later than October 23rd, 2026. Class members should refer to this website for updated information about the final date to submit claims.
There are two ways to submit your claim form:
1. An online claim form emailed to you through Docusign.
OR
2. Printable/Email claim form:
Under United States federal law (28 U.S.C 1746), you understand that by submitting and executing a claim form, you thereby certify that all provided information is truthful and correct under the penalty of perjury.
Do not contact the Clerk. Please do not write or call the Court, the Court Clerk's office, or the settling Defendants with questions about the action or the claims process.
